| Church Equipment Check-out | If you need any equipment for your event please feel out the form below. Any consumables used like video tapes, gas for van, etc. must be purchased by the ministry checking out the equipment. |
| Event Activity Form | Please fill out this form for any event at Restoration Church whether it is held at the church or off site. Please fill out all required questions. (Any non-church event will be charged a $50 fee.) For weddings call the church office for price list. (Submit this form at least 1 month prior to the event.) |
